Attached is the paperwork required for participation in the 2025-26 Eagle Regiment Fall Marching Season.
PHYSICAL PAPERWORK:
We will collect the Member Agreement, Participation Waiver, and Physical Form on in paper format.
The member Agreement, and Spot Deposit are due by May 15, 2025.
Students can hand this paperwork into Mr. R, Mr. B or any volunteer at rehearsal.
Please remember you have to update your Physical every school year. New Members should have their physical handed in by the start of Band Camp 1, on June 4, 2025.
In addition to the Registration paperwork, Member Agreement, and Participation Waiver I have included the OTC Medication form. If you would like your child to be able to take any medications during camps, rehearsal, or travel without us having to call you each time, please fill this form out and return. This also has to be updated yearly.
DIGITAL PAPERWORK:
We are asking for registration information, and emergency contact information to be filled out digitally this year. Additionally, there are 7 Acknowledgements you will need to complete. Parents and Students should complete this digital paperwork together.
There is an option to print these documents at the end of the form.
